I give permission for my child to attend the SHIFT Teens' FUGE Summer Camp from July 7-11, 2025, at 2001 W. Seminary Drive, Fort Worth, TX 76116. I understand that the cost is $175 per camper and that all payments must be completed by May 25, 2025. I also acknowledge that my child will be under the supervision of SHIFT Ministry leaders and FUGE staff during the trip.
